Internship-For-Credit application materials:
Steps For Completing an Internship-For-Credit Application
  1. Complete and sign the Student Section.

  2. Ensure the Internship Field Supervisor completes and signs the Employer Section.  (The employer will also need to provide a job description, which includes the intern responsibilities.)

  3. Identify a faculty sponsor to oversee your internship.  Develop a written Educational Plan in cooperation with your faculty sponsor and submit a copy with the application.  (A sample educational plan is available in the application materials above).

  4. Complete the Faculty Section with the required signatures of you, your Faculty Sponsor, your Faculty Sponsor’s Department Chairperson, and your Academic Advisor.

  5. Visit Career Services located at 619 College Avenue to obtain a signature acknowledging the application’s completion. This must be the last signature you acquire.  Your application will not be signed without a copy of your Educational Plan and a job description from your employer.

  6. Submit your completed application to the Registrars Office by January 31, 2012.  Please note:  You are responsible for ensuring that the Registrars Office receives all of the required information.

Upon completion of your internship, the internship supervisor should complete the Internship Supervisor Evaluation form.  This form must be returned to your faculty sponsor 2 weeks before the end of the semester.
