Insurance, Costs and Fees

Health insurance is required for F&M students. All students are charged a Health Services fee with their tuition.  Students are seen free of charge at Appel Health. Fees occur for vaccines (at cost), rapid strep and mono tests and urine dip.

Medical Health & History Form

The confidential medical and health history form is required to be completed and returned by the due date. Failure to successfully complete health forms may result in your inability to register for classes.
