Where To Start

The New Student Information Form (for students entering Fall 2014)

New students entering in Fall 2014 will receive a welcome email message, sent to their personal email account, that contains an individual link to this web-based form.  If you very recently made your decision to attend F&M, you should expect to receive this email message within one week of sending your matriculation deposit.  If you have not received this message and believe you should have, please contact us at (717) 358-7184 or .

Because the information collected in the New Student Information Form is needed for the Beginnings program (May 12, 14, and 16) and related academic advising, it is very important to complete the form as soon as possible, NO LATER THAN Monday, May 5, 2014.



Other Important Forms

All new students must complete the Student Health Form and return it to Appel Health Services no later than June 15, 2014.  One option for returning the form is to bring it with you to Beginnings, and turn it in at the Health Services table in the Information Fair.  Click the link below to visit the Health Services web site, where you will find the student health form as well as important meningococcal information.  For your convenience, a paper copy of the health form has been mailed to your parents as well.

In addition, parents will receive the Family Information Form by mail, which can be returned in hard copy form to our Parent Relations office, or completed electronically.



Attending Beginnings

Your welcome email message will contain a pre-assigned date that we would prefer you attend the Beginnings Program (May 12, or 14, 2014).  However, if you are unable to attend on your pre-assigned date but are available on one of the other program dates, you may select the date that works best for your family when you complete the New Student Information Form.  Additionally, if you are unable to come to campus for either of May 12 or 14, you may opt to participate in the online, Friday, May 16, offering of Beginnings.



Unable to come to campus for Beginnings

While we would prefer to see all students on campus for Beginnings, we understand that long distance and schedule conflicts may prevent some students from attending the program.  For this reason, we have created "virtual Beginnings" which will be available starting Friday, May 16.  "Virtual Beginnings" will contain an online version of almost all things that occur at the on-campus Beginnings.  Students and parents who are unable to attend on-campus Beginnings for any reason will enjoy this alternative.

Even if you are unable to attend Beginnings, it is still very important that you complete and submit the New Student Information Form ASAP, NO LATER THAN Monday, May 5, 2014.
