Letter of Recommendation Forms
If you wish to apply to law school through F&M, you must complete the following Letter of Recommendation (LOR) forms using the instructions provided by LSAC, the pre-law advisor, and on the forms themselves.
The Letter of Recommendation Request must be completed and given to each individual from whom you are requesting an LOR. It is only required if you are applying to law school through F&M. The second form is a sample of the LOR form required by LSAC. You must print and complete an LSAC LOR form for each individual from whom you are requesting an LOR directly from your LSAC account.
Transcript Request Form
Your must submit the form below to the Registrar in order to have an official F&M transcript sent to LSAC.
