Click on the following links to download the forms that you must complete and submit to various campus offices. The forms highlighted in bold are required of all students. Others may be required, depending on your individual circumstances.

• Liability Release Form
  This form must be completed and submitted to the Office of International Programs before your departure.
• Emergency Contact Form
  This form must be completed and submitted to the Office of International Programs before your departure.
• Course Credit Form
  This form must be completed and submitted to the Office of International Programs, with the appropriate signatures, before your departure.
• Off-Campus Study Expense Planner (from the Office of Financial Aid web site)
  If you receive need or merit-based financial aid, you must complete this form and submit it to the Office of Financial Aid by the deadline indicated for the semester you will be off-campus.
• Program Evaluation Form
  Once you have returned from your off-campus study program, we ask that you complete an evaluation of the program so that we know what it has been like for you and to offer advice to future students.
