Your household has been selected from a random sample of residents in Lancaster County to participate in a survey about attitudes, beliefs and experiences related to mental health and addiction. The results will help community groups develop needed services and programs.

The survey will take about 10 minutes to complete. Your responses will be kept strictly confidential and will only be used for research purposes.

The survey is being conducted on behalf of Lancaster County Joining Forces, a group of local individuals and organizations working together to reduce deaths from overdoses and support people living in recovery from addiction.
