Find the list of approved medical providers to support an employee who needs a medical evaluation or treatment due to a workplace injury or illness.

Step One: Manager First Report of Injury Form

When an incident occurrs that involves employee injury/illness, a manager must complete this form within 24 hours of the incident.

Step Two: Manager Accident/Near Miss Investigation Form

This  is a secondary step to the completion of the Manager First Report of Injury Form and needs to be completed within 72 hours of the incident. This form can also be used to report a near miss incident.
