PA Clearance Forms for Volunteers

Pennsylvania law requires all volunteers working with minors to complete the following forms:

  • Pennsylvania Child Abuse History Clearance 
  • Pennsylvania Access To Criminal Record Check (PATCH)

Any volunteer who is not a resident of Pennsylvania for 10 years must also complete: 

  • FBI Criminal History Check (fingerprinting.) The results will be mailed to the address you provided on the form in approximately two weeks after the fingerprint scan is completed. Please plan ahead!

NOTE:  The Ware Institute also requires copies of your completed clearance forms to be on file at the Ware Institute before you begin your volunteer service.  Please bring the forms to the office in Harwood Commons or email the scanned copies to the Ware Institute before you begin your volunteer service.

Pennsylvania Child Abuse History Clearance 

This form must be completed by you online using the PA Child Welfare Portal.  Select "Individual login" and follow the step-by-step instructions.  A social security number is not required for this clearance, however, the results can be instantaneous if you do provide your social security number. There is no charge for this clearance if you are a volunteer.  

Pennsylvania Access To Criminal Record Check (PATCH)

If you are volunteering at an agency or organization that requires a PA Criminal Record Check, this form should be completed by you online at the PA Access to Criminal History (PATCH) website.  To get started, choose "New Record Check" (volunteers only) and carefully read the prompts and complete the information.   There is no charge for this clearance if you are a volunteer.  A social security number is not required for this check.


Federal Bureau of Investigation (FBI) Criminal Record History Check (Fingerprint)

This form is necessary if you are not a resident of Pennsylvania.  A social security number is required for this check.

PLEASE READ ALL THESE INSTRUCTIONS about how to complete the form, what IDs are required*, including transportation needed to get your fingerprint check done BEFORE you click on the link below.
  • Visit

  • Enter Your Service Code: 1KG6ZJ.  This code is required to correctly identify you as a volunteer in PA and connect your registration to  the Department of Human Services (DHS)

  • Next, select the option at the top of the list "Schedule or Manage an Appointment".  This screen leads to the actual registration appointment form. Complete the form. 

  • When you get to the section about document IDs that are acceptable, choose ONE of the documents in the drop-down list.  Note: You MUST have this original document with you when you get your fingerprints done. It must be the actual document, no copies are accepted.

  • Eventually, you will be asked to put in your address. Please use your COLLEGE ADDRESS and include your CAMPUS BOX NUMBER, if possible. This is so the form is mailed directly to you, not to your parent or guardian's home. 

  • The next screen asks for your zip code in order to find the nearest location. Use 17603 as the zip code.

  • The closest location will be in Willow Street, PA. It is a 15-minute drive from campus.

  • Click on the location, which will take you to the appointment selection page.

  • Make note of the accepted forms of payment at the site you've selected (generally a credit or debit card is required). 

  • You will pay for the clearance when you arrive for your appointment - The cost is $23.85

  • DON'T FORGET to TAKE along the IDENTIFICATION you've selected.


  • You only get ONE CHANCE to open the link containing your clearance results. 
  • Don't use your phone to access the results link.  You will not be able to download the results.  Use a laptop to open the link and download those results. 
  • If you don't, you will have to pay for another record check!
Reimbursement for Cost

In order for the Ware Institute to reimburse you for this expense,  you must bring the following to the Ware Institute offices in Harwood Commons. Please email Lisa Wolfe if you have any questions.

  • Your receipt of the $23.85 you paid for the service.
  • Your actual completed FBI fingerprint form or a .pdf of the form can be emailed to
  • You will not be reimbursed without the original form or a .pdf of the original form and your IdentoGo receipt.

NOTE:  Plan ahead! This process can take three weeks AFTER your fingerprints are scanned. The results will be mailed to the address you provide on the form, so it is a good idea to use your campus address. You must bring a copy of the results or a .pdf of the results to the Ware Institute for our records.  The original must be shown to the agency or school where you are volunteering.

If you have been a resident of PA for the past 10 years or more, you should complete the PA Disclosure Statement in lieu of completing the FBI fingerprint clearance. Please download and print the following form to complete it:
